The RCM program takes a holistic approach, addressing both medical needs and safety concerns including fall prevention, wandering prevention, medication management, and vital sign monitoring. Alerts from monitoring devices are sent directly to Toronto Grace's 24/7 call centre, where a multi-sectoral care team provides prompt escalation and coordinated response.

Developed in partnership with GRT Health and Ontario Health at Home, the RCM program has extended to 20,000 homes across Ontario over three years. The program uses comprehensive triage systems, care planning, and case management to ensure seniors have round-the-clock access to coordinated support for in-home care requirements.

    We will always be deeply grateful for the incredible care and compassion shown by palliative care nurse Allison who supported our family on the final night of our brother’s life. During such an emotional and difficult time, she brought a sense of calm, dignity, and kindness that made all the difference. Allison was not only attentive to his needs, ensuring he was comfortable and at peace, but she also took the time to gently support our family by offering reassurance, understanding, and genuine empathy when we needed it most. Her presence turned an incredibly painful time into one filled with compassion and grace. It’s clear Allison is truly dedicated to the work she does, and her ability to care so deeply for both patients and families is something truly special. The hospital is incredibly fortunate to have someone like Allison on their team. We will never forget the comfort she brought us. Thank you from the Power Family. ❤️

    My wife Jenny has been privileged to be a patient at The Toronto Grace Health Centre which is conveniently located at Bloor and Church, Yorkville - close to the Bloor and Yonge Subway. The hospital offers very specialized services and Jenny was transferred from Mount Senai Hospital following an accident to take advantage of their extensive Rehabilitation Program. The hospital has a dedicated team of rehabilitation experts including a physiotherapy program which has assisted Jenny to become more independently Mobiie. Jenny was also referred to Dr. Zaheen at his Respiratory and Pulmonary Clinic located conveniently in the hospital where Dr Zaheen has been very instrumental in her recovery. The Nursing staff who are all highly skilled professionals are friendly, compassionate, caring and respectful, and the hospital has a relaxed atmosphere in a very pleasant, comfortable and safe environment. Thank you to Dr. Rush and his staff for the exceptional care Jenny received, exceeding all our expectations.
