The team provides consultation in palliative symptom management and complex end-of-life care, advanced palliative care education, and coaching and mentoring of health care professionals.
The interprofessional team of nurse practitioners, registered nurse specialists and physicians is available 24 hours a day, 7 days a week to assist with patients who have life-limiting diseases.
Referrals are accepted from primary care providers, community hospitals with no or limited palliative care services, long-term care homes, retirement homes and other congregate care settings, home care coordinators and service providers, and hospices.
